Need is a critical concept in international health systems because it drives health care policy, development, and delivery at the population and individual levels. Needs assessments are essential to ensure that health services are still required and to identify new target populations with unmet needs. The concept of need is supported by various theoretical definitions drawn from multiple disciplines. However, little, if any, detail of the theoretical or conceptual basis of what is being measured is ever articulated when needs are assessed or health interventions are developed based on need. This can potentially be problematic, as it could render measurement invalid and planned health services ineffective in meeting needs.
Theoretical definitions of need are rarely compared and contrasted. This critical review aims to fill a gap in the literature. Interpretations of the need concept are introduced, drawing on psychology, social policy, and health. The concept and significance of unmet healthcare needs are discussed. These definitions are intended to operationalize the term “need,” theoretically drive needs assessment, and help guide need-based health care decisions.
