Evaluate Global Healthcare Delivery Models
ANSWERS
It has become crucial to describe, assess, and carry out research on the issues brought up by comparative effectiveness research and characterize care delivery organizations of all sizes, from small independent individual provider units to massive integrated health systems. The Delivery Systems Committee of the Effective Health Care Stakeholders Group of the Agency for Healthcare Research and Quality, which represents a wide range of perspectives on health care, recognized this challenge and developed a draft framework with domains and elements that may be useful in characterizing various sizes and types of care delivery organizations and may contribute to key outcomes of interest. The framework might open the door to more research in areas without precise definitions and descriptions.
The need for standardized techniques to characterize, measure, compare and assess delivery system changes has grown among stakeholders due to recent and ongoing innovation in systems for delivering and reimbursing health care in the United States. The quality of decisions and care for patients, providers, researchers, policymakers, payers, and other stakeholders may be improved by excellent communication and transparency on these changes, which would be made possible by a standard taxonomy of delivery system features. According to the Institute of Medicine, comparative effectiveness research (CER) is “the generation and synthesis of evidence that compares the benefits and harms of alternative methods to prevent, diagnose, treat, and monitor a clinical condition or to improve the delivery of care.” Comparative effectiveness research is a top priority. Although they range from independent individual provider units to substantial integrated health systems, there is no common descriptor for care delivery units or systems that captures their breadth. As a result, it is difficult for stakeholders to assess the generalizability of a study or an innovation implemented in one location because there is no universal language for describing delivery systems. Whether the environment is a vast integrated care system or a small independent practice, and whether clinicians are paid on production or a salaried basis, the effectiveness of an intervention may be substantially different in each case. In order to help stakeholders in the healthcare industry better understand, assess, disseminate, and implement delivery system innovation in a more informed, transparent, and stakeholder-centered way and to enable comparisons between them, we propose a preliminary framework for describing healthcare delivery systems. Our goal is to present the framework’s domains and components, the processes used to create them, and examples of how they could be utilized in various contexts.
