The five main core competencies and functions that should be discussed include; quality improvements in the organization’s processes, application of evidence based practice, provision of patient-centered care, inter collaborations within interdisciplinary teams and utilization of informatics. Their objective is to ensure the smooth running of the health care facility and improve on the quality of health on persons aged 55 and above within the community.

The improvements in the quality of services delivered involves a careful analysis of the health care facility’s basic safety standards and identifying any possible errors noticed in the handling of dangerous and hazardous material. Dealing with aged persons, the GAH and surrounding hospitals should ensure that they work together in such a way that the people can be referred to either of the facilities without worry of the quality of service. Through the use of health care informatics, these organizations can stay updated on the latest medical trends and new research finding on disease management strategies.
Utilization of Informatics
In the modern society, nursing and medicine have integrated healthcare provision with advanced communication technology in ensuring efficiency is achieved in receiving and transferring important data (Ginter, Duncan & Swayne, 2018). Through well-developed organization systems, mainly in form of software, GAH Hospital and other health care facilities within the region can share vital information on diseases and conditions affecting the aged people within their areas. Access of data from a common database will help in providing previous patient information, necessary for improving on the accuracy of treatment.

i. Budget control should be done to ensure that quality improvements are carried out in time and when necessary.

Competencies for JCAHO Accreditation
To achieve accreditation by the Joint Commission On accreditation of health organizations (JCAHO), GAH Hospital needs to achieve competencies according to the standards set by the commission. According to Runciman, Merry & Walton, (2017), these include; high standards of patient care, exceptional medical and clinical knowledge depending on the facility, high levels of interpersonal skills especially in communication skills, professionalism in carrying out all functions that affect the quality of service delivered, improvements in practice based learning and the ability of management to adopt informatics through a system based practice.
Integration of IT Systems
Due to the relatively moderate size of the clientele the hospitals serve, there should be introduction of a healthcare information system for the new GAH to be constructed, while the introduction of a health informatics system to be shared by the facilities should also be developed. The health care information system will be used as a database to store patient and employee information and also provide easy access of patient information within the different departments; this streamlines health care provision. As a larger and long term project, the health informatics system should be created with regards to the set Health Insurance Portability and Accountability (HIPAA) guidelines that dictate on the integration of ethics in service delivery on health care IT systems.
System Requirements

The function of the HIPAA is to basically protect the patient by ensuring that the privacy, security and safety of their information is guaranteed (Runciman, Merry & Walton, 2017). Health information and informatics systems are technologically developed platforms where data is fed, stored and can be retrieved from. Just like other computer systems, they can be easily accessed and manipulated by the people with access to them. The HIPAA has made it its duty to ensure that all health care staff is fit to handle and manage such information without leaking it to the public or allowing loopholes for manipulation through hacking.

The system is required to collect with ease and store safely the patient’s information as given by them. The system is also expected to be safe and secure in that it is not easily hacked, although accessible by the other health facilities. This means that its structure should be designed in a way to protect patient information from being accessed by unauthorized persons. With banking, social security or national identity card important in signing in a patient, the system should also be set in a way that this information is kept strictly private and should only be shown to the mandated investigative bodies upon request (Review CMS, 2016).

Strategic planning and management is an aspect of business development that should be incorporated by an organization in order to achieve growth and expansion. Application of proper planning techniques such as budget formulation and organizational structure development allows the teams involved to work on a plan. The organization should focus on maximizing the patients’ experience at the facility to encourage returning patients. This works to increase revenues.
It is important that the Mission Hospital, Community Clinic and GAH work within the set time frame on the budget on how and when to complete the development of this new unit. Incorporating the proper technologies, while observing the ethical considerations, would be a necessary move in striving for growth and expansion of the hospital’s services.
